Temporomandibular joint disorder, or TMD, affects over 10 million people in the United States. It limits jaw function, drives chronic headaches, and reshapes how the entire upper body holds itself together. When it goes unaddressed, the dysfunction spreads — into the neck, the shoulders, the face, and the way you sleep.

At Jawmatrix, we evaluate TMJ disorders as a structural problem, not just a dental one. Our approach combines advanced imaging, clinical assessment, and the proprietary Jawmatrix Method to identify and address the full system driving the dysfunction.

What is the temporomandibular joint?

The temporomandibular joint (TMJ) is the hinge that connects your jawbone to your skull, located just in front of each ear. It is one of the most complex and frequently used joints in the body — it allows you to speak, chew, yawn, and move your jaw in every direction.

When the TMJ is functioning properly, you don’t notice it. When it isn’t, the symptoms can be persistent and debilitating.

Common symptoms of TMJ dysfunction

• Jaw pain or tenderness, especially when chewing or yawning
• Clicking, popping, or grinding sounds when opening and closing the mouth
• Difficulty fully opening or closing the jaw
• Locking of the jaw in open or closed position
• Headaches, particularly in the temples
• Ear pain, ringing, or a feeling of fullness without infection
• Facial pain, neck pain, or shoulder tension
• Morning jaw soreness or evidence of nighttime grinding (bruxism)
• Changes in how the teeth fit together when biting

What causes TMJ dysfunction?

TMD rarely comes from a single source. The most common contributors include:

• Cervical and postural strain — forward head posture, prolonged screen time, and cervical instability all alter how the jaw rests and functions
• Soft-tissue and disc displacement — when the disc inside the joint shifts out of position, popping and clicking sounds develop
• Bruxism — chronic jaw clenching or teeth grinding, often related to sleep dysfunction or sympathetic nervous system overdrive
• Inflammatory or degenerative arthritis — including age-related changes, autoimmune arthritis, and joint inflammation
• Trauma — direct impact to the jaw, whiplash from motor vehicle accidents, or sports injuries
• Aberrant condylar tracking — abnormal motion patterns of the mandible visible on dynamic imaging
• Stress and autonomic dysregulation — chronic sympathetic activation drives masseter and temporalis tension

How Jawmatrix evaluates TMJ disorders

A precise evaluation is essential for effective treatment. We use diagnostic tools and clinical assessment that go beyond what most TMJ practices can offer.

Dynamic Digital Radiography (DDR) — when indicated

DDR captures motion at up to 15 frames per second, at low radiation dose, revealing how joints and surrounding structures actually function during movement. Depending on clinical presentation, Dr. Pezeshkian may order TMJ DDR to assess condylar tracking and joint mechanics, cervical DDR to evaluate the contribution of the upper neck, or both. The decision is made at the time of evaluation based on what the clinical picture indicates.

DDR is the only imaging tool of its kind in California, and it allows us to see TMJ dysfunction in ways static X-rays and MRIs cannot.

Comprehensive structural assessment

A thorough clinical exam evaluates cervical alignment, jaw range of motion, masseter and temporalis tension, postural contributors, and craniomandibular system findings. This identifies which factors are driving your specific case.

Treatment — the Jawmatrix Method

The Jawmatrix Method is our proprietary, multi-modal approach to TMJ dysfunction. It draws on Dr. Pezeshkian’s dual training in chiropractic and dental hygiene, over a decade of TMJ-specialized clinical work, and continuous integration of advanced assessment and treatment techniques developed for the craniomandibular system.

Each patient’s plan is built from the ground up based on evaluation findings. What’s used, when, and in what sequence is determined by your specific clinical picture — not by a generic protocol applied uniformly to every patient.

This is a different model from short-cycle pain management. The Method is designed to produce structural change, not temporary symptom relief.

When TMJ disorder warrants referral

We do not work in isolation. For patients whose evaluation indicates needs outside our scope — including oral appliance fabrication, surgical consultation, or formal sleep medicine diagnosis — we coordinate with established specialists. Each provider working at the top of their scope produces better outcomes than any single provider trying to do everything.
